APPLICATION CHECKLIST

To: All HHA’s, CNA’s, LPN’s, RN’s

The following documents are required to register with our company and must be current and up-to-date:

• Resume
• Florida Driver’s License or Florida Identification Card
• Social Security Card
• Employment Authorization
• Professional Licenses (RN, LPN, CNA, HHA)
• CPR Card (Red Cross, American Heart Association, others)
• Auto Insurance Card/Proof
• Liability Insurance (can be applied upon hire)
• Physical Exam Must be completed within the last 2-3 months by an MD or Nurse Practitioner and state “free and clear from all communicable diseases.” Note: Physicals by chiropractors are not accepted.
• TB Test or Chest X-Ray
• Level 2 Background Screening
• Certificate from School (for CNA and HHA only)
• CEUs (Continuing Education Units) 12 hours of annual in-service required
• Original 4-Hour HIV Certificate Alzheimer’s/Dementia Certificate
• HIV Update Certificate
• OSHA Update Certificate
• HIPAA Update Certificate
• Domestic Violence Update Certificate
• Medical Errors Update (for LPNs and RNs only)

Please Note: If you cannot provide employment verification for the last three months, a new Level 2 Background Screening will be required.
